How to request W-9 Form

Use the form below to request a copy of Zaptiva’s W-9 for your records.

To protect sensitive information, W-9s are only issued after we verify the requestor. Once you submit the form, we will validate the email address provided against our records. If the email cannot be successfully validated, the W-9 will not be sent.

This step ensures W-9s are shared only with authorized parties and prevents accidental or unauthorized disclosure.
